Member Forms

Due to statutory limitations for minors, any individual conducting business with Farmward must be at least 18 years of age. A minor may be part of a joint account as long as the other account holder is at least 18 years of age and is fully liable for the obligations entered into with Farmward. If the prospective customer is a legal business entity, then the principal (majority) owner of the entity must be 18 years or older and must be a guarantor. This limitation applies to both purchases of products and services as well as sellers of products and services.

Prior to Farmward’s extension of credit, applicants must complete the following forms:

• Credit / Member Application and Credit Agreement
• W-9
• Certificate of Exemption (“ST3”) if applicable
• If purchasing agronomy or feed products, the applicant must provide a detailed financial statement (balance sheet) with any appliable schedules dated within six months of the Credit Application.

In addition to the documents above, if the applicant is a Corporation, Partnership, or Limited Liability Company, then the following documents are required:

• Corporation: Certification of Shareholders and Officers for Corporation
  Personal Guarantee
• Partnership: Certificate of Partners and Managing Partners for Partnership
  Personal Guarantee
• Limited Liability Company: Certificate of Members and Officers for Limited Liability Company
• Personal Guarantee: An inquiry may be made through a credit reporting agency to determine creditworthiness. Farmward reserves the right to deny or limit the extension of credit.

Farmward’s billing cycle starts on the first day of the month and closes on the last day of the month.

• Statements are mailed or emailed on the first business day of the month following the billing cycle. Statements are also available in Farmward’s customer portal, Grower360.
• Statement balances are due in full no later than the 15th of the month.
• If a payment is not received by the 15th of the month, a finance charge equal to 1.5 % (18% annually) shall be applied against the unpaid statement balance. The 1.5% applies to not only the current outstanding balance but all previous unpaid balances as well.

NSF checks and ACH returns will be assessed a $25.00 service charge.

Farmward has a priority lien on any equity and patronage held or owed to a member or patron. If the member/patron’s account is past due and is eligible to receive an equity redemption or cash patronage payment, Farmward shall apply any such payment to the past due account.
